Registration

In order to join an HTCNE Medical Teams Abroad mission, all team members must complete the registration process. Your prompt action is appreciated as there is much that our office must do in order to ensure the team's approval to work in country and secure the best airfare possible.  Should you have any questions, do not hesitate to contact Missy Law at 860 355 1828 x24 or Georgann McCarroll at 860 355 1828 x23.   Forms are available in .pdf or Word format - just click on the appropriate link below

Step 1)  Download and complete the registration form & release from liability. Return via snail mail - HTCNE, PO Box 129, New Milford, CT 06776, Fax -  860 350 6634 or email mlaw@htcne.org or georgann@htcne.org

Step 2) Download and complete the Member Information Package, which includes the credit card authorization form and Medical History Health Form.   Return via snail mail - HTCNE, PO Box 129, New Milford, CT 06776, Fax - 860 350 6634 or email to mlaw@htcne.org or Georgann@htcne.org

Step 3) Send via snail mail - HTCNE, Po Box 129 New Milford, CT 06776. Fax - 860 350 6634 or email mlaw@htcne.org or georgann@htcne.org a copy of your resume and license to practice. If you have traveled with HTCNE in the last 12 months, we may not need this information. If your license has expired or you have changed positions or place of employment since you last traveled, please send updated information

Step 4) Scan or take a photo of the photo page of your passport and send via email to mlaw@htcne.org or georgann@htcne.org. If you are not able to do either, please make a photo copy and send via snail mail  - HTCNE, Po Box 129 New Milford, CT 06776 . WE DO NOT ACCEPT PASSPORT PAGES BY FAX. If you send us your passport photo page via fax, we will only ask you to resend it.
