Steps for Applying to the
Domestic Kids Program
The following is an
outline of the steps involved in applying to the Healing
the Children - Northeast Domestic Kids Program.
Completed applications must include:
1) A fully completed Program Application 2) Photos: We request one photograph of the child’s affected
area, if it is a visible medical condition, accompanied by
one photograph of the child’s face to be used for identification purposes. 3) A completed Medical Authorization Form
Although we would
like to help all children that need assistance with
medical treatment, our program does not financially
provide for care. We arrange donated or discounted
medical treatment and assistance to children living
within Connecticut, New York, Massachusetts,
Rhode Island, Vermont, New Hampshire and Maine.
Acceptance into our program is financially based, for
children that do not qualify for insurance or are
under-insured. Parents are required to disclose
full financial status in the application.
Specific services
provided are medical and surgical treatment,
prescription drugs, dental care, eye care, medical
equipment and supplies such as adaptive equipment and
assisted technology, physical therapy, occupational
therapy, and speech and language pathology.
Much like our
International Inbound Program, doctors,
nurses and hospitals donate their facilities and
expertise in order to assist children eligible for the
program.
ELIGIBILITY
REQUIREMENTS:
An
application
must be completed in full and submitted to HTCNE to be
reviewed. Once the application has been submitted
in full, it is reviewed by the program director and
program coordinators.
The family will then be
contacted by a case manager about further proceedings.
The child must reside
within the limits of our territory and meet our age
requirements. Age restrictions for the program are
birth to eighteen years old.
Due to our limited
resources, we generally are not able to accept
applications for terminal or life-threatening
situations, including organ transplants (except skin and
cornea), stem cell and bone marrow transplants, and most
cancers.
RESPONSIBILITIES OF
THE CLIENT ONCE ACCEPTED INTO THE PROGRAM:
1. If there
are any changes concerning financial income, insurance,
address, phone number or status of any kind, we are to
be notified immediately.
2. It is the client’s
responsibility to find transportation to and from
appointments. If they are unable to find
transportation, it is their responsibility to contact
HTCNE to arrange transportation.
3. The client is required to keep their scheduled
appointments. If they cancel
or fail to show up to a scheduled appointment, without
good reason, they may become ineligible for further
assistance.
4. It is required that a client get permission from
their case manager to make an appointment with a
doctor’s office for their child. A client cannot go to
the doctor’s office on their own. HTCNE requires
that the child’s case manager approve and make all
appointments.
5. In an emergency, a
child may be brought to a local emergency room to be
treated by the hospital, without prior permission. When
the hospital’s bill for services is received by the
client, the original copy must be submitted to HTCNE, so
that we may speak with the billing department about the
possibility of waiving fees.
Our programs run on a yearly cycle. If a
case remains inactive for one year, a new application
must be submitted in full.
